Purpose

This form can be used to report all aviation safety occurrences. Accidents and serious incidents must be notified to the CAA as soon as practicable.

The Civil Aviation Authority collects information for the purposes of enhancing aviation safety.

The information submitted in this form is being collected in accordance with Part 12 of the Civil Aviation Rules.

Any information submitted by you will held by the CAA and will be used or disclosed only for lawful purposes in accordance with the Privacy Act 1993 and/or Part 12 of the Civil Aviation Rules, or as otherwise required or permitted by law.
